Monitoring and Supervision of Adoptive Placements

Scope of this chapter

The prime responsibilities or the monitoring and supervision of adoptive placements will be outlined in the local procedures within the local authority where the child was in care. Refer to the local procedures of each member authority of One Adoption South Yorkshire Regional Adoption Agency. The regional adoption agency will work in partnership with the child's agency to support pre-adoptive placements through their family finders and the adopters' social workers.

  1. The adoption support plan agreed at the match of the child with the adopters will be a key document in supporting adoptive placements. This should be reviewed as part of the overall placement review process, chaired by the IRO. The placing agency and One Adoption South Yorkshire are responsible for ensuring the plan is put in place. It will updated to reflect changes in need;
  2. Visiting patterns and contact with the family will be included in the Adoption Placement Plan. These should be agreed by the child’s worker and the adopters’ social worker, to be clear of roles and maximise the effectiveness, as well as meeting statutory requirements. Visiting patterns and contact should be kept under review according to need and joint visits should be carried out as required;
  3. Visits made by the adopters’ social worker will be recorded on the adopters' case record;
  4. The adopters’ social worker is responsible for ensuring that OASY initial and core adoption support offer is understood by the adoptive family and will support the adopters in accessing these;
  5. Where there are concerns about the risk of a placement breaking down, then a review meeting should be organised immediately by the child’s social worker and attended by all relevant parties;
  6. The procedures to be followed where a placement disrupts are set out in Disruption of Adoptive Placements Procedure;
  7. At the point of adoption order the adopters social worker will continue to offer support to the family for 12 months post order. This will be a minimum of quarterly contacts.
